The before-and-after stories are compelling, but have left many wondering:

What a GLP-1 Does

Semaglutide is marketed as Ozempic for certain adults with Type 2 diabetes and as Wegovy for chronic weight management and other approved indications.

It works by mimicking a naturally occurring gut hormone called GLP-1, which signals the brain to reduce appetite, slows gastric emptying (meaning food moves more slowly through your digestive system), and helps regulate blood sugar and insulin response.

What Happens When You Stop Your GLP-1

GLP-1-based medications directly affect hormonal and neural pathways involved in appetite and glucose regulation. However, they do not permanently eliminate every biological, behavioral, or environmental contributor to obesity. When treatment stops, appetite suppression and other medication effects diminish. Because the body commonly resists sustained weight loss, many patients regain some or most of the weight.

A 2026 systematic review of weight-management medications found that weight regain after medication withdrawal was faster, on average, than regain after behavioral programs. The review included several medication classes, and long-term GLP-1-specific follow-up remains limited. Based on the observed rate of regain, researchers estimated that participants could return close to baseline weight within roughly 18 months, though longer direct follow-up is limited. Other health benefits like lower blood pressure and improved cholesterol reversed on the same timeline.

Because obesity is often chronic and weight regain is common after treatment stops, many patients may need ongoing medication or another long-term weight-management strategy. Patients should consider the practical implications of long-term treatment, including cost, route of administration, adverse effects, monitoring, and personal preferences.

The Financial & Physical Costs of Long-Term GLP-1 Usage

  • Cost: Without insurance, Ozempic and Wegovy can run $1,000 or more per month. Insurance coverage is inconsistent and sometimes denied for weight loss (as opposed to diabetes). Oral incretin-based options are becoming available, but price and insurance coverage vary and should be verified using current prescribing and coverage information.
  • Side effects: Nausea, vomiting, constipation, diarrhea, acid reflux, and abdominal cramping are commonly reported side effects, particularly during dose escalation. For some, these are significant enough to discontinue treatment.
  • Muscle loss: As with other forms of substantial weight loss, some lean mass may be lost during GLP-1 treatment. Preserving lean mass is important for strength and metabolic health. Adequate protein intake, resistance training, and individualized clinical monitoring may help preserve muscle.
  • Nutritional deficiencies: Some patients may have difficulty meeting protein, fiber, fluid, or micronutrient needs because of reduced appetite or adverse gastrointestinal effects. A clinician or registered dietitian can assess individual risk.

Medication-Supported vs. Lifestyle-Based Weight Management

Medication-supported and lifestyle-based approaches use different tools. Both require individualized care, and some patients benefit from combining them.

Whether the weight loss is achieved through habit shifts in a person’s nutrition and activity or through medication or metabolic surgery, it is important to supplement with behavioral support, adequate sleep, and professional guidance.
